I was in a café today and I could hear a woman singing. No, there was not some sort of open "mic" thing. Nor was she was a scheduled performer. Yes, I was wearing headphones. No, I do not believe that she has any serious mental difficulties.

I don't think there are any etiquette rules about this. Because, seriously, who does that? You are not in your house!
I like the Cure. I probably don't like the Cure as re-imagined by you. I almost certainly don't like the Cure as re-imagined by you cutting in on Jonathan Agnew telling me about England's latest batting collapse.
It is a public place! You owe people the courtesy of letting them pretend you are not there!

Eh. Move to South side Chicago. You'll get people singing all the time, but usually it will be someone belting out a possibly improvised song about Jesus and/or going to heaven to meet Jesus. You'll miss the coffee shop person.
Here here. I can handle a bit of humming or whistling, but outright singing in public is too much. The first thing I think of is, like you mentioned - 'does this person have mental problems?' And maybe she doesn't have an abnormally low IQ and hasn't been diagnosed with a mental handicap, but I think there is something up in this person's head. She should have enough self-awareness to understand that what she's doing is a bother to some people and that she's being impolite in ignoring that fact. It's like bringing your yappy little dog or playing your crap music in public - no one wants to hear it.
